Senior Assessment/Geriatric Evaluation
An innovative program designed to address the needs and concerns of older adults
Services Provided
Services can be accessed by family members, physicians, community organizations,
or by self-referral. The interdisciplinary team can address a number of issues that
may be of concern to older persons and their families, such as:
- Physical limitations
- Emotional and behavioral changes
- Difficulties with independent living
- Need for nursing home placement
- Alternative living arrangements
- Evaluation of medications
- End-of-life issues
SAGE Clinic services may include physician referrals, physical or occupational therapy
referrals, X-ray tests, laboratory exams, and pharmacy services.
The SAGE Team
The SAGE team is composed of a nurse practitioner who visits with patient and family
in the home environment to assess safety and other issues; a physician who determines
the patient’s medical needs through a detailed history and complete physical exam;
a social worker who assesses the patient’s needs and, if necessary, makes recommendations
or referral to community services; and a dietitian who completes a nutritional assessment
to evaluate the quality of the patient’s meal plan.
Payment for SAGE Clinic Services
Medicare and some supplemental insurances cover services at the clinic. The clinic
staff will provide assistance in filing health insurance claims.
The SAGE Evaluation
The evaluation takes approximately three hours. A family member or close friend
should accompany the patient to the evaluation. When possible, medical records and
all current medications, including non-prescription medications, should be brought
to the evaluation.
